Contact & appointments
Choose your next step.
Register for your visit, contact the practice, arrange a referral or plan your follow-up.
Emergency? Arrange care now.
For a suspected medical emergency, do not wait for a form, callback, email or chatbot reply. In South Africa, call 112 from a mobile or 10177 for an ambulance, or go immediately to Netcare Olivedale Hospital A&E or the nearest emergency department. Outside South Africa, use the local emergency number.
Registration & appointment enquiries.
Use the registration form to provide the information requested for practice registration. If you need administrative help with registration or an appointment enquiry, use the contact options below. Submitting the form does not mean that a clinician has reviewed your information and does not confirm an appointment. The practice must confirm appointment arrangements separately.
The registration form requests identification and contact details, funding information and relevant health information. Please read Privacy and your information before entering personal information. For questions about a required field or help completing registration, contact the practice.
Opens Microsoft Forms in a new tab. Read how your information is handled.
The callback form is for appointment and practice administration enquiries. Enter contact details only; do not include clinical information. Submitting a callback request does not mean that a clinician has reviewed it, does not confirm an appointment, and does not guarantee that an appointment can be secured.
Callback and administrative enquiries are monitored Monday–Friday, 08:00–17:00 SAST, excluding public holidays and announced closures. We usually respond within 24–72 hours. Submissions outside these hours or during a closure may take longer. This is not an emergency service.
If the form does not open or you do not receive confirmation, call the practice. Registration alone does not confirm an appointment. For an emergency, use the emergency contacts above.

Appointment callbacks are overseen by Dr TT Mahlobo. An enquiry or registration does not guarantee an appointment. Any appointment offered must be confirmed separately by the practice.
Planning for your visit
Questions for your medical scheme or fund.
Ask the practice for the proposed service details and any documents needed before contacting your medical scheme or healthcare fund.
What should I ask before a consultation or scan?
- Does my plan cover the specialist consultation and vascular ultrasound or duplex scan separately?
- Do I need a referral letter, prior authorisation or a designated network provider?
- Which practice, hospital and procedure details do you need?
- Can you confirm the answer, authorisation reference and validity dates in writing?
What should I ask before a procedure?
Check the proposed procedure and hospital with your fund. Ask which referral documents, authorisation references and provider details are needed, and confirm the validity dates with your fund.

Privacy and your information
Responsible party: Dr TT Mahlobo Inc. This information covers the current online registration and administrative enquiry routes used by My Veins Vascular, Dr TT Mahlobo's practice.
Our Information Officer is the IT Director. Contact director@myveinsvascular.com for privacy enquiries. See privacy-enquiry response times and contact guidance.

What we collect and why
Depending on the form, we ask for identification and contact details, medical-aid or other funding information, and relevant health information. We use these details to register you, respond to your enquiry, support care coordination and carry out related practice administration. A referring clinician or an authorised representative may provide information on your behalf.
Our short website callback form asks for your name, email address, an optional telephone number and a contact category. It is for administrative requests; do not include medical records, ID documents, photographs or a clinical history there.
Please provide only information relevant to your request. A form may require particular fields before it can be submitted. If you are unsure why a detail is needed, or cannot provide it, contact the practice for assistance. Missing contact or registration details may prevent us from responding or completing registration. A required form field does not itself mean that a law requires you to supply it.
Where information goes and who can access it
The registration form uses Microsoft Forms. The public website and the separate administrative callback form use Squarespace. For questions about how your registration information is handled, including storage and authorised access, contact the Information Officer at director@myveinsvascular.com. See privacy-enquiry response times and contact guidance.
The practice owner receives submission notices. A separate administrative alert is configured for the practice owner and the Communicate mailbox; it does not include form answers or provide access to them. Clinical or administrative staff may be given access where authorised for your care or enquiry, limited to what they need for that work. A saved record or notice does not mean that a member of the clinical team has reviewed it.
These services may process information outside South Africa. Contact the Information Officer if you need details about the services used or how your information is handled. Please ask the team to arrange an appropriate secure route before sending clinical documents; do not send them through the callback form, ordinary email or public chat.
How long information is retained
The practice's stated policy is to retain website enquiries and registration records for ten years. This is a policy period, not a promise that information is automatically deleted after ten years. You may ask how the policy applies to your information or request a review of information that no longer needs to be retained.
Access, correction and privacy concerns
You can ask the Information Officer about your information, request access or correction, raise an objection, or request deletion where applicable. Legal conditions may affect a request. Describe your request briefly and ask for secure verification instructions before sending identity documents or medical records.
You can also lodge a privacy complaint with South Africa's Information Regulator or contact the Regulator on 010 023 5200.

Registration and urgent care
Submitting a registration form does not mean that a clinician has reviewed your information, does not confirm an appointment, and does not guarantee that an appointment can be secured. The practice must confirm appointment arrangements separately. Open registration form (opens Microsoft Forms in a new tab).
For an emergency, call 112 or 10177, or attend Netcare Olivedale Accident and Emergency or the nearest Accident and Emergency department. Do not wait for a form, email or online reply.
Privacy enquiries
For a question or concern about your personal information, contact the IT Director at director@myveinsvascular.com. This address is for privacy enquiries only.
Due to high enquiry volumes and triage, responses may take between 24 hours and 10 days.
For all other enquiries, use the relevant channels on this website: clinical, appointment and administrative enquiries. For urgent symptoms, follow the emergency guidance without waiting for an email reply.
Please describe your privacy request briefly. Ask for secure document-sharing instructions before sending identity documents or clinical records.
