Prepare the information below, then use the secure transfer route agreed directly with the practice.
- Reason and urgency
State the referral question, relevant symptoms, their duration and any deterioration. Discuss urgent cases directly by telephone.
- Clinical context
Include relevant diagnoses, examination findings, comorbidities, allergies, medications and previous vascular procedures.
- Investigations
List available vascular studies and laboratory results, with dates. Arrange access to the actual imaging as well as reports where relevant.
- Coordination and feedback
Provide your professional contact details and preferred secure reply route. Confirm receipt, the next clinical step and how the consultation outcome will reach the referring team.
